The worker lost his balance and fell, when he had finished the pruning of a part of one group of the branch the pine, and he got off the ladder to do back part of branch.
Incident The victim carried out pruning of the one group of upper ( about 3.7m - 5.0m heights ) branch of the tree 90 minute after starting work, and when he was start to next work, he lose his balance and fell from the eighth stage ( about 2.3m heights ) of the ladder onto a concrete alley way, got a hard knock on the head and died.
Sequence The pruning work of the tree which was in the garden a private house was carried out by a worker of the leader ranking and the victim.
After they arrived to client's residence the moving, they made arrangement about 10 minute. By arranged, the victim was doing pruning work of a pine at foreyard near the porch and the other doing pruning work of garden plants at the back yard.
The leader worker told the victim to wear a helmet and a safety belt while working.
The victim carried out pruning of the one group of a branch part ( about 3.7m - 5.0m heights ) over the tree for about over 90 minute.
The victim was working without using a safety belt or wearing helmet at that time.
When moving, he lose his balance and fell from eighth stage ( about 2.3m heights ) of a ladder onto a concrete alley way, got a hard knock on the head and died.
Cause Despite the fact that there was a danger of a crash at the pruning work which was carried out about 2.3m height from the ground, there was no crash prevention measures such as installing safety belt to the tree of pine, etc.
