Scenario
Case Name Oil pressure cylinder of the aerial vehicle was damaged, and a workman(electrician) falls from the bucket.

Overview In the bird damage countermeasure construction field , the victim(electrician) changed the 2 bind wires of road side on the backet of aerial vehicle.After that, he made the vehicle down once,and drew the electric wire , intending to start his work from the parking opposit to the road.Then ,the"Link A" under the 1st and 2nd boom of the vehicle was broken and it fell suddenly to the parking with the sound "bang", then he fell down to the parking.
Incident Around 5pm, in the bird countermeasure construction field of utility poles,the victim(electritian)was on the backet of an aerial vehicle , and did a repairation work. During it, the "Link A" under the 1st and 2nd boom was broken and the backet fell suddenly, he fell down to the parking.
Sequence Around 13:30, the victim arribed at the construction field of bird countermeasures on the aerial vehicle (with movable boom and for 1 person ride.).
The arrangement of the work was that the victim for the repairation of electric wire ,the chief of the group for observation, and group members for handling the materials.
Around 17:00, to repair the wire of the 3rd pole, he made the boom down once after the restoration of he bind wire of the top of the pole, then he thought to start his work from the parking opposit to the road . Then, the "Link A" under the 1st and 2nd was broken , so the backet in which he rode suddenly fell down from the height of 8.5m to 1.5m, with the sound"Bang", he fell down to the concrete ground,and died.
Cause The parts welded on the LinkA and boss suddenly broken.
The cause of the sudden break was thought that the LinkA was not welded well and got rust as time passed then the crack was made.
The monthly and yearly inspection fixed by law was not done and not before the work.So the cracks were looked over.
The victim did not use the safety band "D-formed", so he was driven to fall down to the ground.
Though he picked up the " falldown from the backet " in the prediction of danger practice, he did not execute the actual countermeasure of it.
Response Inspection and repairation of the vehicle was done , cracks of it and broken parts were fixed.
Countermeasures As the inspection method of vehicles, yearly ,monthly ,and before-work inspection fixed by the law must be executed surely,and make sure that there is not any crack and broken point and dirty point . And, if any abnormality is found, repairation ,and other maintainance must be done.
Make the list of the special self-inspection-cars and make the records of the inspection and its results.

Do the safety patrol on a certain intervals, and make texts of the checkpoints.
Check the usage of the safety bands and make sure the points derived from danger prediction .
To the workers, make them sure to do works according to the safety manuals.
Background It was misterious that the disorder of the vehicle was not recognized at all .
Sequel Inspection of all aerial vehicles for the lead-in were carried out.
