Overview In arch bridge erection field in the Kagoshima prefecture, 2 of the 4 lifting lug of anchor which fixed the wire to the P2 bridge pier fell off with the anchor during construction of the wire bridge for scaffold between leg P1 and P2 as temporary facility before the arch bridge erection.
Therefore, the wire bridge tilted, and 3 of the 5 workers on the wire bridge fell, and 2 of the 3 died and another received the serious wound.
Incident During wire bridge construction, 2 of the 8 lifting lug of anchor which fixed the wire fell off with the anchor, and the wire bridge tilted, and 3 workers fell.
Sequence * The installation of the wire bridge was started on the previous day of the accident.
* The anchor was fixed during the day, and mounting of the wire and installation of the safety guard were carried out.
* They worked to install the steel pipe the wire on the day. At around 9:38, 2 of the 8 anchors of the P2 bridge pier came off, and the wire bridge greatly tilted.
* 3 of the 5 workers who worked on the wire bridge fell into the bottom of a ravine. 1 worker who used the safety belt escaped the fall, and 1 person already clung to the net.
* 2 of the 3 falling people died, and 1 person received the serious wound.
Cause * The anchor was the metal expanded anchor, and the hole of the anchor had been opened at the size according to the regulation, but the cone had remained in the hoop of anchor hole, and the expansion division did not extend sufficiently, and the strength was not enough.
* The reason why the anchor base did not expand may be the failure of the construction placing shortage or placing forgetfulness or the concrete shavings etc.
* The manager does not recognize the importance of the anchor, and the confirmation of whether the construction was surely done or not.
Response * After the construction restart, the adherent anchors (chemical anchor) were used.
* They established safety belt mounting facility in which the human did not fall, even if the wire bridge fell.
Countermeasures * The chemical anchor must be used as the anchor of the wire bridge.
* The manager is present in the construction of the anchor, and confirms the construction situation.
* The load test should be carried out even if it is a temporary construction.

* Though it was clarified in "The criterion of the construction of scaffold protecting work in steel bridge construction work" to use the chemical anchor as the anchor fitting for the wire bridge, it was not adopted in this field.
Sequel A commissioner with the bridge construction safe committee said that it was proven that the wire bridge is installed in such a construction. It is a method to install an embedding anchor in "substructure construction ."
